If you have purchased or received a FAMACHA© chart between the dates of August 10, 2006 and October 1, 2006, please check and make sure that the lamination has not caused premature fading. Faded charts were accidentally distributed and can change the accuracy of scoring.

If you are uncertain about the integrity of your chart or feel that your chart has been compromised due to lamination, return to the following address and a new chart will be provided to you free of charge.
